Dear participant,

The Public Health Nutrition Department at Zayed University invites you to fill the following questionnaire. This study is divided into two parts:

• Part 1: Nutrition assessment, questionnaire.

• Part 2: Food taste assessment through university cafeteria visit, where the meal will be measured, and participants would be asked about food taste and preferences.

You have been randomly selected to participate in this study. Your participation is valuable and voluntary. You can withdraw from the study at any time and without providing any reason. All data is confidential and remains stored in a locked cabinet during the study and destroyed after completion. Only the authorized person can access the questionnaire's digital copies as they are password protected. The name and telephone number will be used EXCLUSIVELY to call the participant and arrange for the laboratory visit. Therefore, please answer these questions because all the information will be uniquely used for the research study.

There are no correct or false answers. We appreciate that your answers reflect your lifestyle.

I have read the explanation about the study. I was allowed to discuss my queries and questions, and they have all been answered. Accordingly, I agree to sign up for this study and realize that it is voluntary, and I have the right to withdraw from the study whenever I want.
